Fraternization Policy: Students may not socialize with teachers unless the studio is participating as a function. Notice to Customer: You are entitled to a copy of the agreement at the time you sign it. Keep it to protect your legal rights. Do not sign this agreement if it has any blank spaces. I certify that I have received a completed and signed copy of this member(s) agreement and of any other document which I have signed. This represents the entire agreement between buyer and Anytime Dance Studios. I further certify that I have read all pages of this agreement prior to affixing my signature and understand and agree to all terms and conditions as stated. Compliance with Rules: I understand and agree that a 24/7 studio membership is a special membership based on trust and is a privilege, which can be taken away for a violation of rules. As a studio member, I agree to abide by all membership rules and 24/7 membership rules, which will be posted at the Studio, on the Studio website, and may be amended from time to time at the sole discretion of the Studio. The additional rules below apply to a 24/7 membership: 24 Hour Access to Studio: You are able to enter the studio at any time throughout the day with proper conduct. You agree to abide by all policies, guidelines, rules, and regulations for safe use of the studio, including following any verbal or written instructions provided by the studio's owners and instructors, and not endangering any other member. Anytime Dance Studio Rules are subject to change. YOUR RESPONSIBILITY FOR DAMAGE You agree to pay for any loss or damage to Anytime Dance Studios or the Studio caused by your willful or negligent act, or as a result of your breach of this Agreement. • Only one 24/7 member may enter the studio at a time during non-staffed hours. Everyone must enter individually. • KEY CODE sharing is strictly prohibited and will result in immediate loss of membership; key code sharing is viewed by ownership as stealing services. • Only active account members will be allowed entry. • Pre-approved 24/7 members under the age of 18 must be accompanied by an approved member parent until they reach the age of 18. • Improper unauthorized use of the Studio may result in member suspension or cancellation. No one may use your access code for any reason, and you are required to report any situation that appears to be code sharing to Studio staff. • The Studio reserves the right to suspend or cancel the rights, privileges and membership of any member whose actions are detrimental to the use, safety, and enjoyment of the Studio. • No Studio access during schedule Studio Class and Socials • Do not bring or otherwise allow a person that is not an approved member or guest of the Studio to access the facility without permission. • Do not bring or otherwise allow a person younger than 18 years of age to access the Studio without permission. • Students alone in the Gym should call 911 in case of an emergency.
